Request permanent TwinSpires self-exclusion
The current TwinSpires self-exclusion article says a person seeking permanent exclusion must speak with a Player Services supervisor at 1-877-774-7371. After that conversation, a Permanent Exclusion Form can be mailed to the address on file.
Balance and activation details are not published
The reviewed instructions do not state when permanent exclusion becomes effective, what happens to a balance or open wager, or whether a withdrawal can continue after exclusion. Ask the supervisor in the same call, but do not delay an urgent protection request solely to resolve those questions.
Call Player Services
Call 1-877-774-7371 during the published Player Services hours of 9:00 a.m.–10:00 p.m. Eastern Time. Say that you need a supervisor for a permanent self-exclusion request.
Request the current permanent-exclusion form
Ask the supervisor to confirm the current process and send the Permanent Exclusion Form to the address on file. If the address is no longer current, ask for the secure address-correction route.
Ask about access and open items
Ask when the exclusion becomes effective, what account and wagering access it blocks, and what will happen to the current balance, open wagers, pending transactions, and account communications.
Return the form through the instructed route
Complete only the current form and return it through the route the supervisor confirms. Keep a copy, the case reference, the submission date, and written confirmation of the effective date and scope.

New York and Iowa have separate state programs
The TwinSpires article points New York residents to the New York Voluntary Self-Exclusion program and Iowa residents to the Iowa Racing and Gaming Commission self-exclusion program. Those state programs are separate from the TwinSpires operator form. Check the current state instructions for their scope and enrollment rules; this guide does not imply that one request automatically creates the other.
