How to enroll
- Use the official DCP online portal, or follow the current paper form’s secure-upload, postal, or notarized submission instructions.
- Only the person seeking exclusion can enroll. Complete identity verification through the official state route.
- Do not treat a submitted request as active until DCP confirms enrollment.

Ending or removing the exclusion
One- and five-year entries do not end automatically. After the selected term, request removal through the official portal and complete identity verification. A lifetime state exclusion cannot be reversed under the current terms.
Before you submit
- A licensee may apply a broader policy to affiliates, out-of-state services, properties, rewards, or entertainment, but that broader scope is not guaranteed by the state list.
- Use the separate property program below when you also need on-reservation exclusion from Foxwoods or Mohegan Sun.

Ending or removing the exclusion
The current form describes the five-year selection as automatically rescinding after its term and the permanent selection as not removable.
